If you have Medicaid coverage you need to contact your caseworker, or the Medicaid
office, to determine if the claim for this account was already submitted. They have
representatives who should be able to answer

your questions, and give you a status
on why payment has not yet been made.
* Note: Medicaid does have a filing limit, so please be sure to investigate any
discrepancies with your carrier promptly. In the event they have no record of this
claim, we will assist you by submitting a claim on your behalf. You will need to
contact our office to give us the necessary information. This may be easiest by
sending us a copy of Medicaid card. Or you can call and ask for an insurance billing
representative. We will need your name, address, phone number, and the Medicaid
ID# for the patient. Please have all information available when you call.
