If you are 65 or older, have end stage renal failure, or are permanently disabled,
you probably have Medicare Benefits. Please contact Medicare to determine if this
claim was filed, why it has not been paid at this time, and get a status on when
a check will be sent to the provider.
For Your Information:
Medicare Part A provides benefits for In-patient hospitalizations. Medicare part
B provides benefits for Out-patient services such as; ambulatory surgery, pre-surgical
testing, physician services, and diagnostic testing.

Your Medicare card will indicate if you have Part A or Part B, or both, and their
effective dates. If you are 65 or over, but you or your spouse are still actively
employed, your employer group health plan may be the primary carrier for medical
claims. In this case you would need to be sure that the claim was submitted to the
primary carrier prior to Medicare.
If the Medicare has no record of this claim, we will assist you by communicating
this information to the hospital, or doctor's office, and requesting that the claim
is re-submitted. Please call and ask for an insurance billing representative. We
will need your name, address, phone number, and the Medicare #. Please have information
available when you call.
